About Sacred Care Transitions
Family members and friends are often thrust into the role of hospice caregiver, a sacred responsibility that comes with little preparation and many unknowns.
Who We Are
Sacred Care Transition was born out of one of the most tender seasons of my life. On April 21, 2025, my dear mother-in-law, Linda Lou Mealing, passed away peacefully at home under hospice care. Walking alongside her during her final weeks was both sacred and deeply emotional, a time filled with love, faith, and moments our family will cherish forever.

Out of that experience, we created a simple toolkit to help us stay organized, prepare her home environment, and ensure she received the dignity, compassion, and love she so richly deserved. That humble toolkit became the heart of Sacred Care Transition, a faith-based resource designed to guide families and caregivers through the emotional, spiritual, and practical aspects of end-of-life care.
Beyond this personal journey, my professional path has always been rooted in service and compassion. In serving in the community for over 20 years, I’ve had the honor of working with older adults and supporting families through the challenges of dementia care and aging. I co-founded We Care Connect Memory Café, a community program offering caregivers and individuals living with memory loss opportunities for social connection, creative engagement, and mutual support.
As a program manager, I helped pilot Healthy Together, a volunteer and student-led initiative in partnership with the University of Michigan School of Nursing and Packard Health. This program focused on improving the health and wellness of older adults in the West Willow neighborhood, one of many ways I’ve been blessed to serve throughout Washtenaw County, Dearborn, and surrounding communities.

Through Sacred Care Transition, my hope is to extend that same spirit of care and connection to families navigating life’s most tender transitions.
